While we do not provide case management services directly, we work closely with each member’s assigned case manager to ensure their care plan is followed and their needs are met. This collaboration helps coordinate services efficiently and supports the overall well-being of the individual.

House cleaning services support individuals who have difficulty maintaining a clean home environment. This may include tasks such as vacuuming, dusting, laundry, dishwashing, and general tidying to ensure the living space remains safe and comfortable.

Personal care services assist individuals with daily activities such as bathing, dressing, grooming, and toileting. These supports are designed to promote hygiene, dignity, and overall well-being, especially for those with mobility or cognitive challenges.

Transportation services help individuals maintain their independence by providing reliable rides to medical appointments, grocery stores, adult day programs, and community events. This service is essential for those who are unable to drive or access public transportation safely.

Safety checks are regular visits or phone calls conducted to ensure an individual's well-being. These checks may include visual assessments of the home environment, confirming the person is safe and responsive, and addressing any immediate concerns.

Supported community living helps individuals live independently within the community through services such as life skills training, supervision, and personal development. This support is tailored to promote self-sufficiency while ensuring the individual’s health and safety.
